When it fires

The upstream model rejected the request as malformed. This is a platform-side bug — your input passed our DTO validation but the shape we forwarded didn’t satisfy the upstream. Logged at ERROR so engineering gets paged. Any held credits are released.

How to recover

Retry with backoff — the bug is being investigated. Open a support ticket with the response request_id so we can correlate to the exact log line.
